Benign hepatic masses

良性肝腫瘤の中で圧倒的に頻度の高い疾患は嚢胞と血管腫である。いずれも典型的な画像を呈した場合には診断に迷うことはないが，出血を反復した嚢胞や染まりの遅い血管腫など非典型的な像を呈することもある。また，その他の良性肝腫瘤は多血性の像を呈して肝細胞癌との鑑別が問題となることがある。これら良性肝腫瘤は悪性疾患とは治療方針が異なるため，正確な画像診断を行うことは臨床的な意義が大きい。診断に難渋することもあるが，診断の手がかりとなる特徴的な画像所見を正確に拾い上げることが重要である。本稿では良性肝腫瘤の中で特徴的な画像所見を呈する疾患について症例とともに概説する。なお，本稿で提示するMRI画像はすべてEOBを使用したものである。

Benign hepatic masses can be difficult to differentiate from malignant lesions. Since the treatment strategy for benign hepatic masses differs from that for malignant lesions, accurate imaging diagnosis is quite important. To make a correct diagnosis, it is important to accurately pick up the characteristic imaging findings that provide clues to the diagnosis, for example, central scar or high signal intensity on hepatobiliary phase of focal nodular hyperplasia, atoll sign of inflammatory hepatocellular adenoma, early venous return of angiomyolipoma, and double-target sign of hepatic abscess. CT and MRI are useful for the diagnosis of benign hepatic masses, thus radiologists can play an important role in diagnosing them.
