Myocardial CT perfusion assessment

虚血性心疾患においては，冠動脈の形態的な狭窄評価だけでなく，心筋灌流評価など機能的評価が治療方針の決定や予後評価に重要であることが知られている。我が国においてもその重要性は高まってきており，平成30年度の診療報酬改定に伴い安定冠動脈疾患に対して待機的に施行する経皮的冠動脈インターベンションについては，原則として術前検査などにより機能的虚血の存在が示されていることが算定要件に加えられた。現在，single photon emission computed tomography（SPECT），positron emission tomography（PET），magnetic resonance imaging（MRI）が心筋灌流画像評価として主に使われており，その有用性については数多くのエビデンスが蓄積されている。

In coronary artery disease, myocardial perfusion assessment is important for the decision of treatment strategy and prognosis. Recently, myocardial CT perfusion has emerged as a novel non-invasive myocardial perfusion imaging. Myocardial CT perfusion imaging allows for identifying myocardial ischemia with high diagnostic performance. Additionally, Myocardial CT perfusion imaging simultaneously provides both morphological information of coronary artery and functional information of myocardial perfusion by integration with coronary CT angiography in a single cardiac CT study.
