Diagnosis of myocardial ischemia using MRI

負荷心筋血流MRIを用いると心筋虚血の有無や程度が評価でき，冠動脈狭窄の機能的重症度を高い精度で診断できる。近年，欧米を中心にエビデンスレベルの高い研究が実施され，負荷心筋血流MRIを実診療レベルで実施しようとする機運が高まっている。本稿では，負荷心筋血流MRIを用いた心筋虚血診断の方法やその診断能などについて解説し，安定冠動脈疾患患者の治療方針決定における負荷心筋血流MRIの意義について考察する。また，心筋血流MRIによる心筋血流定量解析の虚血評価における有用性や将来展望について述べたい。CTやSPECTなどと対比し，負荷心筋血流MRIの長所や問題点についても考察したい。

Stress perfusion MRI can be used to assess the presence and extent of myocardial ischemia and diagnose the functional severity of coronary artery stenosis with high accuracy. In this article, I will describe the methods and diagnostic capabilities of stress perfusion MRI for the diagnosis of myocardial ischemia and discuss the significance of stress perfusion MRI in determining the treatment strategy for patients with stable coronary artery disease. In addition, the usefulness of quantitative analysis of myocardial blood flow by MRI in the assessment of ischemia and its future prospects will be discussed.
