下垂体卒中の2例

Two cases of pituitary apoplexy

下垂体卒中とは下垂体の血管障害にて，突然の頭痛，視力障害などを引き起こす症候群である^1）。下垂体卒中をきたしやすい基礎疾患として，ほとんどの場合に下垂体腺腫を認める^2）。下垂体腺腫の卒中には出血と梗塞があるが，両者の画像所見と術中所見，病理所見を対比した報告は少ない。今回，下垂体腺腫の出血を伴わない梗塞例と出血例の2例を経験し，画像所見と術中所見，病理所見を対比検討することができたので，若干の文献的考察を加えて報告する。

Pituitary apoplexy is a fatal medical emergency due to hemorrhage or infarction of pituitary gland. Pituitary apoplexy usually occurs to pituitary tumor, adenoma. The diagnosis of pituitary tumor apoplexy can be difficult because hemorrhage can change over time in CT and MRI images. Furthermore, pituitary adenoma infarction usually results in coagulation necrosis which shows T1WI high intensity and T2WI low intensity. We report two cases of hemorrhagic pituitary tumor apoplexy and non- hemorrhagic pituitary tumor apoplexy with radiologic-pathologic correlation.
