前頭蓋底発生脊索腫様髄膜腫の1例

A case of anterior skull base chordoid meningioma

脊索腫様髄膜腫（chordoid meningioma）は1988年にKepesら^1）によって初めて報告された髄膜腫のまれな亜型であり，2016年のWHO脳腫瘍分類ではgradeⅡに分類されている。まれな亜型であるため画像的な特徴は不明な点も多い。今回我々は診断に苦慮した前頭蓋底発生脊索腫様髄膜腫の1例を経験したので，文献的考察とともに報告する。

A 23-year-old man presented with nasal obstruction for 3 years and a headache for a few days. Head CT and MRI revealed an anterior skull base mass with intra- and extracranial extensions. The mass showed homogenous enhancement and intratumoral prominent flow voids. After gross total resection, the tumor was diagnosed as chordoid meningioma. We think it is difficult to distinguish chordoid meningioma with few mucoid matrix from other meningiomas/SFT.
