腎腺腫症を背景に発生した乳頭状腎細胞癌の1例

A case of papillary renal cell carcinomas derived from renal adenomatosis

腎乳頭状腺腫は腎皮質から発生する良性腫瘍で，画像で遭遇する機会は少ないものの，剖検例では比較的発見頻度が高い。通常単発でサイズも小さく臨床的に問題となることはほとんどない^1-3）。一方，まれな病態として通常両側性に腎乳頭状腺腫が5個以上多発することが報告されており，腎腺腫症と呼ばれる^4）。このうち核異型の低い乳頭状腎細胞癌が同時に存在した症例では腎乳頭状腺腫と乳頭状腎細胞癌の病理像に類似性が認められており，一連の病態である可能性が提唱されている。今回，2個の1型乳頭状腎細胞癌と腎腺腫症を合併した症例を経験したので報告する。

64 years old male was found to have two left renal masses. The masses showed gradual enhancement on dynamic CT, hypointense on T2-weighted MR images, and hyperintense on DWI. Multiple tiny lesions similar to simple cysts are also noted on the left kidney. Histopathologically, two masses were diagnosed as renal cell carcinomas and multiple tiny lesions as micropapillary adenomas more than 40. The renal cell carcinomas and adenomas showed morphological similarity, suggesting that the carcinomas derived from the renal adenomatosis.
