免疫放射線療法―放射線療法と免疫療法との併用療法

Immuno-radiotherapy：combination of radiation therapy and immunotherapy

がん免疫療法への注目が，近年，かつてないほど高まっている。2011年にBeutler博士，Hoffmann博士，Steinman博士が，自然免疫系の活性化や，免疫応答における樹状細胞の役割を解明した功績によりノーベル生理学・医学賞を受賞し，2013年には，免疫チェックポイント阻害薬の臨床的有効性が評価され，Science誌のBreakthrough of the yearに「cancer immunotherapy」が選出された。さらに，2018年には，Allison博士と本庶博士が，cytotoxic T-lymphocyte antigen-4（CTLA-4）受容体およびprogrammed cell death-1（PD-1）受容体の発見と，その抗腫瘍免疫応答への機能解明の功績により，ノーベル生理学・医学賞を受賞した。そして今，医療の現場において，免疫チェックポイント阻害薬をはじめとしたがん免疫療法は，少なくとも一部の疾患・状態では，「標準治療」としての地位を確立している。さらに，かつて，分子標的薬と化学療法や放射線療法の併用について多くの臨床試験が行われたように，今，世界中で，免疫療法と放射線療法や化学療法との併用療法の臨床試験・治験が数多く試みられている。特に，免疫療法と放射線療法の併用は，単なる組み合わせではなく，理論的に相乗的・相補的な意味をもつ。本稿では，免疫療法と放射線療法との併用に関する理論的背景，現状，今後の展望について述べる。

The interest in cancer immunotherapy has increased more than ever in recent years. Cancer immunotherapy, including the application of immune-checkpoint inhibitors, has become a “standard treatment” for several malignancies. Recently, a numerous number of clinical trials of combinations of immunotherapy, radiation therapy, and chemotherapy has been conducted worldwide. Notably, the combination of immunotherapy and radiation therapy may demonstrates not simply an additive effect but theoretically synergistic and complementary effects. In this article, we summarize the theoretical background, current status, and future prospects of the combined use of immunotherapy and radiation therapy.
