脂肪濃度を含む多発リンパ節腫大をきたし特徴的画像所見を呈したアミロイドーシスの1例

A case of amyloid lymphadenopathy presenting multiple enlarged lymph nodes with diffuse fatty density

アミロイドーシスとは，現在30種類以上知られている可溶性のアミロイド前駆蛋白が，生体分子との相互作用による構造変化，重合を経てアミロイドと呼ばれるβシート構造をもつ不溶性の異常蛋白に変異し，全身諸臓器の細胞外に沈着して臓器障害を引き起こす疾患群の総称である^1）。アミロイドーシスの画像所見はしばしば非特異的で多様なため，画像のみで診断に至ることは難しく，病型や病変の局在によっては悪性を否定できず手術を施行される例も存在する^2-4）。

The imaging findings of amyloidosis are often non-specific and diverse；thus, diagnosis by imaging is often challenging. We report a rare case with a striking CT finding of multiple lymphadenopathies with diffuse fatty density. We suspected amyloid deposition as the cause of the characteristic lymphadenopathy, according to a previous report. Surgical biopsy confirmed the diagnosis. Though little has been reported on radiological findings, the unique imaging feature may be a key to the diagnosis of amyloid lymphadenopathy.
