膵充実性腫瘍における良悪性の鑑別

Tips and pitfalls in the differentiation between benign and malignant solid lesions of the pancreas

各臓器において，病変の良悪性診断は極めて重要である。膵においては膵癌の診断が最も重要で，これは予後が不良であるためである。膵癌は膵の充実性腫瘍では最も代表的な疾患で，血行動態の観点からは乏血性病変に分類される。乏血性病変には悪性腫瘍だけでなく，良性腫瘍および炎症性疾患などが含まれる。一方，多血性病変においても良悪性病変が含まれる。本稿では，乏血性および多血性病変に分類したうえで，良悪性病変の特徴および鑑別に際するポイントについて概説する。

It is very important to differentiate between benign and malignant pancreatic solid lesions. In a process of differential diagnosis, solid lesions can be categorized into hypovascular and hypervascular lesions. The major candidate for hypovascular lesion is pancreatic carcinoma, and the major candidate for hypervascular lesion is pancreatic neuroendocrine tumor. It is occasionally difficult to differentiate them, and this results in unnecessary treatment such as surgical resection or inappropriate steroid therapy. To avoid these problem, we should to know the tips and pitfalls for differentiation between them.
