胆道病変における良悪性の鑑別

CT/MR diagnosis of biliary system：differentiation between benign and malignant lesions

胆道・胆管は十二指腸乳頭部直上から肝内末梢胆管・毛細胆管まで広く分布する臓器であるが，その発生は大きく上流（末梢）側（肝内胆管）と下流側（肝外胆管）で異なる。本稿では肝内胆管については肝の項に譲り，肝外胆管と胆嚢の疾患について論ずる。まず胆道の発生の概略を述べた後，胆嚢・胆管の悪性腫瘍の大部分を占める腺癌の画像所見のバラエティの広さとその意義について考察し，それらと画像上鑑別すべき良性疾患を取り上げ，その類似点および鑑別点について整理することが目的である。その際，我々放射線科医の現在の一般的画像手段であるCT/MRIを用いたアプローチとして，①形態からのアプローチ，②血行動態からのアプローチ，③MRI拡散強調像からのアプローチの大きく3つの観点から述べてみたい。

Tipis and tricks for differentiation between benign and malignant lesions of the gallbladder and extrahepatic bile duct are presented using CT/MRI. After dividing lesions into three major morphological categories, hemodynamic information and MR signal（particularly diffusion-weighted images）information may be applied for the differentiation. Although there are some overlaps, this approach may enable benign vs malignant differentiation of these lesions, with clinical information taken into account together.
