大動脈から分画肺に流入する異常血管と肺動脈の間に高度な短絡形成を認めた1例

A case of an aberrant artery to pulmonary artery in the intralobar pulmonary sequestration

肺分画症は体循環から起始する異常血管により栄養され，正常な気管支と交通しない肺組織を有する病態である。正常肺との境に胸膜を有するものは肺葉外肺分画症，有しないものは肺葉内肺分画症と分類される。肺葉内肺分画症の還流血管はほとんどが肺静脈であり，肺動脈への還流例はまれである。さらに異常血管と肺動脈の短絡形成の報告は少ない。今回，我々は分画症に流入する異常血管と肺動脈の間に短絡形成を認めた症例を経験したので，若干の文献的考察を加え報告する。

A 60 year-old-man was diagnosed as left intralobar pulmonary sequestration. Angiographic finding led us to the aberrant artery returning via the pulmonary vein and communicating the pulmonary artery. We assessed the vessel was connected to the pulmonary artery by inflammation because there was no pulmonary hypertension in histogy. This was a case of a rare type of an aberrant artery in the intralobar pulmonary sequestration.
