両大血管右室起始，大動脈縮窄修復術後の繰り返す大動脈弁下狭窄に対してRoss-Konno手術を施行した1例

Ross-Konnno Procedure for the Patient of Intractable Subaortic Stenosis after Operation of Double Outlet Right Ventricle with Coarctation of the Aorta and Subaortic Stenosis：Report of a Case

はじめに　遠位型（non-committed type）の心室中隔欠損（VSD）を伴う両大血管右室起始症（DORV）の二心室修復においては，術後の左室流出路狭窄（LVOTO）がしばしば問題となる．LVOTOの原因として以下の三つが考えられる（図1）．①VSD孔の狭小化，②routeの狭小化，③大動脈弁輪もしくは弁直下の狭小化である．それぞれに対応した術式の検討が必要となる．われわれは大動脈縮窄（CoA），大動脈弁下狭窄（SAS），大動脈弓低形成（hypoarch）に対する心内修復術後に繰り返すLVOTOに対してRoss-Konno手術を行い，良好な結果を得たので報告する．

Recurrent left ventricular outflow obstruction （LVOTO） after intraventricular rerouting （IVR） with ventricular septal defect （VSD） enlargement and aortic arch repair for double-outlet right ventricle （DORV）, non-committed VSD and coarctation of the aorta was successfully performed by a Ross-Konno procedure at 1 year 6 months as the second reoperation. Ross-Konno procedure could be an effective option in treatment of recurrent LVOTO after IVR for DORV.
