筋弛緩薬が原因物質と考えられたアナフィラキシーの既往のある患者に対する冠状動脈バイパス術の1例

Coronary Artery Bypass Surgery for the Patient with a History of Anaphylaxis due to Neuromuscular Blocking Agent ; Report of a Case

麻酔薬に対するアナフィラキシーの頻度は，正確な値は不明であるが，高くはないと報告されている．しかしながら心疾患は重症化のリスク因子であり，麻酔薬によるアナフィラキシーを発症した場合の死亡率は決して低いものではない．今回，麻酔薬に対するアナフィラキシーのためいったん冠状動脈バイパス術（CABG）を中止し，原因物質の特定などアナフィラキシーに対する戦略を立て，CABGを施行した症例を経験した

We present herein a patient with rocuronium anaphylaxis, which had been identified using skin test, underwent conventional coronary artery bypass surgery without any neuromuscular blocking agent. Immobility was achieved with sedatives and analgesics.
