実体験の語りに基づいた火山噴火災害の急性期に必要となる看護技術と知識の考察

要約

目的：

　本研究は、火山噴火災害の急性期に、被災者に対して看護を実践する際に必要となる看護技術と知識について、看護師の実体験に基づいて考察することを目的とする。

方法：

　2014年に発生した御嶽山火山噴火災害の急性期に、御嶽山の現地もしくは搬送先の医療施設において、被災者に対して看護を実践した看護師10名に半構造化インタビューを実施し、質的帰納的研究デザインに基づいて分析した。

結果：

　【身体面のケアに関する看護技術と知識】と【精神面のケアに関する看護技術と知識】、【火山噴火災害の発生を想定した準備に関する看護技術と知識】という3つの上位カテゴリーと13の下位カテゴリーが見出された。

結論：

　本研究の結果、火山噴火災害の急性期に、被災者に対して看護を実践する際に必要となる看護技術と知識について、【身体面のケアに関する看護技術と知識】では看護師自身が火山ガスや火山灰の吸入を予防しながら火山灰の除染や火山灰・噴石による特殊な外傷や熱傷に対処するための看護技術と知識、【精神面のケアに関する看護技術と知識】では予期せぬ過酷な被災体験によって衝撃を受けた被災者１人ひとりの精神面に寄り添いながら被災者が安心して内面を表出できるための看護技術と知識、【火山噴火災害の発生を想定した準備に関する看護技術と知識】では予期せぬ火山噴火災害の発生と火山噴火災害の急性期を想定した看護技術と知識の修得および資機材の準備・点検、緊急時の看護体制の確認に関する看護技術と知識が必要となることが明らかとなった。

Object:

　This study aims to discuss the skills and knowledge required for acute phase nursing of victims after a volcano disaster based on the narration of true experie nce.

Methods:

　Semi-structured interviews were held with ten nurses who had engaged in the nursing of victims in the field or hospitals after the subacute period of the volcanic eruption of Mt. Ontake, Japan in 2014, and the collected data were analyzed based on the method of the qualitative inductive study design.

Results:

　The analysis yielded 13 subcategories and 3 categories, including “skills and knowledge related to physical care,” “skills and knowledge related to mental care,” and “skills and knowledge related to preparation in anticipation of a volcano disaster.”

Conclusions:

　The findings revealed that 1) removing of volcanic ash contamination and dealing with specific damage due to volcanic ash and cinder while doctors are protecting themselves from the volcanic ash and cinder under “skills and knowledge related to physical care,” 2) consideration of victims' feelings due to the severe and miserable experiences from an unexpected volcano disaster and having victims feel relief and providing them an outlet to confide their feelings under “skills and knowledge related to mental care,” and 3) daily preparation skills and knowledge and confirmation of equipment and the emergency nursing system taking an unexpected volcano disaster into consideration under “skills and knowledge related to preparation in anticipation of a volcano disaster” are the skills and knowledge required for acute phase nursing of victims after a volcano disaster.
