「精神科領域における看護診断導入についての現状と課題」の中間報告

Interim Report of“Current State and Problem of Nursing Diagnosis Introduction in Psychiatry Department Area”

　2007年8月に予定している全国調査の予備調査として，6月に行われた看護診断学会のワークショップ参加者とデイスカッションを行った．そこで話し合われたことを整理分析した結果，次のような結論が得られた．精神科臨床看護職が必要と感じている看護診断ニーズは，①精神症状をもつ人の看護診断が欲しい（たとえば妄想に支配される人の看護問題），②看護過程と看護診断のつながりに関する知識が欲しい，③理論をベースにした根拠のある看護診断が欲しいというものであった．具体的には人間行動をアセスメントするときに根拠のない叙述は患者を傷つけるので，誰にでも説明できる人間行動の根拠が精神科看護には必要である．また，看護診断導入が難しい理由として精神症状を表す看護診断用語が抽象的すぎると考えられた．参加者で最もよく使われる診断名が「思考過程混乱」であったが，それで精神症状のすべてを当てはめることはできないと推察された．

　In August 2007, we conducted preliminary research on the needs of psychiatric nurses who attended the Japan Nursing Diagnosis Academy conference in June 2007. Analysis of their feedback yielded the following results. Nurses require sound nursing diagnosis based on human behavior to help them apply their theoretical knowledge to the nursing process. They need a nursing language that does not have detrimental effects on their patients. The terminology used for describing mental conditions in nursing diagnosis is very abstract for use in the psychiatric field. The term“Disturbed Thought Process”is used most often, but it cannot be applied to all mental conditions.
