要旨:複数の把持形態が可能な5指駆動型の筋電義手であるbebionicハンドを適用した片側前腕切断症例を担当した.bebionicハンドの特性を考慮した集中的な装着訓練を行い,実用的な把持形態が3種類から5種類へ増加した.最も使用した把持形態は母指外転位のカラムであった.使用率をもとに把持形態の設定と切り替え方法を調整することで,筋電義手操作習熟度評価表による習熟度が72.6%から87.3%に向上した.患者立脚型評価法である日本語版ミシガン手の健康調査質問表の患側ADLが40点から65点,両手動作スコアが53点から75点へ改善した.bebionicハンドは,実用性のある代償手段になる可能性がある.
We report on a unilateral transradial amputee who used a bebionic hand which is a myoelectric prosthesis capable of multiple grasp functions. Intensive training was carried out for the application of the bebionic hand considering its characteristics, resulting in the number of practical grasp functions increasing from three to five. The patient was able to perform bilateral actions, and function became easier, with the proficiency level on the Myoelectric Prosthetic Hand Skill Assessment scale improving from 72.6% to 87.3%. Furthermore, on the Japanese version of the Michigan Hand Outcome Questionnaire, the ADL score on the affected side improved from 40 to 65 points, and the bilateral action score rose from 53 to 75 points. The most used grasping function was thumb abduction with the column of the thumb. These results indicate that the bebionic hand could be a practical compensatory measure.
