要旨:Lung volume reduction surgery(LVRS)が,重症肺気腫患者のADLおよびQOLに影響を与えるか否かを,術後24ヵ月にわたり検討した.ADLは術後3ヵ月の時点が最も良い状態にあり,その後徐々に低下していき,術後24ヵ月の時点では術前に近い状態に戻った.一方,health-related quality of life (HRQL)評価では術後に大きな改善はみられなかった.術後,肺機能・運動機能は改善を示したことから,LVRSによる主観的なHRQLへの影響は肺機能・運動機能とは異なるパターンをとることが示唆された.
We studied the long-term (24 months) effects of lung volume reduction surgery (LVRS) with regard to life style and subjective quality of life (QOL) in patients with severe emphysema. The activities of daily living (ADL) level score was highest at 3 months post-LVRS and decreased gradually and returned to the pre-LVRS level at 12 months post-LVRS. In contrast, the health-related quality of life (HRQL) score did not improve significantly. Therefore, it is concluded that the improvement in lung function and exercise performance after LVRS has not been directly connected with the improvement in HRQL in patients with severe emphysema.
