Principles of an Integrated Community Care System and Community-based Rehabilitation

はじめに

　これまで地域リハビリテーション（以下，リハ）の諸活動にかかわってきたが，今回2025年を目途に始められる地域包括ケアシステムは，理念や目標を共有するものであると考える．これまでの地域リハの経験は，地域包括ケアシステムの構築に極めて有用であると思われるため，積極的なかかわりが期待される．

　ここでは，地域リハや地域包括ケアの概要を紹介し，地域リハと地域包括ケアの融合，その推進課題などについて述べる．

Abstract : The knowledge, skills and experiences gained through long years of efforts promoting community-based rehabilitation are useful to develop an integrated community care system. Therefore, it is necessary to promote community-based rehabilitation that works in collaboration with integrated community care. To achieve integrated cooperation among the relevant institutions and facilities in the community, there are three issues to be addressed : the first issue is ensuring cooperation among medical facilities, the second issue is cooperation between the medical facility and the integrated community care facility, and the third issue is cooperation among the integrated community care facilities. Integrated community care teams from the relevant facilities cannot function properly without the activities of the rehabilitation service based in the community. At the same time, it is very important to promote mutually supported activities in the community which incorporate participation of the various facilities and rehabilitation services based in the community.
