はじめに

　胸椎〜腰椎の前方アプローチは1950年代に報告されたHodgsonらのアプローチが有名であり，1990年代まで化膿性脊椎炎，脊柱変形，腫瘍，外傷，骨粗鬆症性椎体圧潰などに数多く施行された^3,4）．2001年にはextreme lateral interbody fusion（XlIF）^7），2012年にはoblique lateral interbody fusion（OLIF）^8）による側方経路からの腰椎椎体間固定が報告され，側方アプローチとして腰椎の前方手術は再度注目されている．側臥位での前方固定術の適応は，外傷，感染症，腫瘍，脊柱側弯症に対する変形矯正術など多岐にわたる^2,9）．前方手術の急速な普及の一方，重篤な合併症の報告も散見され^1），胸腰椎前方手術の安全な施行は喫緊の課題である．手術体位は手術時間や合併症の発生に影響を与える．本稿では，当院における胸椎・腰椎における側臥位手術時の体位のとり方や手術室のセッティングなどについて詳述する．
