腹腔鏡下に摘出した胃穹窿4部の石灰化を伴うGISTの1例

Laparoscopic surgery of a gastric GIST with calcification

　今回われわれは，胃の石灰化を伴うgastrointestinal stromal tumor（GIST）に対して腹腔鏡下胃部分切除術を行ったので報告する．症例は69歳，男性．上腹部痛にて受診した．胃内視鏡検査，腹部CT検査にて胃穹窿部の石灰化を伴う粘膜下腫瘍と診断した．腫瘍は，内視鏡検査にてdelle，bridging foldが認められ，大きさが約3cm大であったため，腹腔鏡下胃部分切除術を施行した，本邦における石灰化を伴う胃GIST報告例は自験例を含め5例であり，腹腔鏡下に切除したという報告は自験例が1例目であった．石灰化を伴う胃平滑筋肉腫症例も合わせて検討し，腫瘍径が5cm以下の石灰化を伴う胃GIST症例に対し腹腔鏡下手術はよい適応と考えられた．

We report a patient having gastrointestinal stromal tumor (GIST) with calcification of the stomach who un-derwent laparoscopic surgery. A 69-year-old man was referred to us for evaluation of the upper abdominal pain. Endoscopic and CT examinations demonstrated a submucosal tumor with calcification at the fornix of the stomach. Because the tumor which, measured approximately 3 cm in diameter, showed ulcer formation and bridging folds by endoscopic examination, we performed laparoscopic partial gastrectomy.
