単孔式腹腔鏡下手術にて切除した胃内発育型GISTの1例

Single-incision laparoscopic surgery for gastric GIST with intra-luminal growth pattern : a case report

◆要旨：患者は52歳，女性．胃内視鏡検査，腹部CT検査にて胃体部小彎に約4cmの胃内発育型粘膜下腫瘍を認めた．手術は臍部の1か所の切開創に3本のポートを挿入して単孔式腹腔鏡下胃局所切除術を施行した．術後は合併症を認めず経過良好であった．病理組織検査では低リスク群のGISTと診断された．単孔式腹腔鏡下手術は疼痛軽減や整容性の点で優れた術式であり，胃GISTに対する低侵襲治療の選択肢の1つとなり得るものと考えられた．

　We report here a patient with gastric gastrointestinal stromal tumor（GIST）who underwent single-incision laparoscopic partial gastrectomy. The patient was a 52-year-old woman. Gastric endoscopy and abdominal computed tomography revealed the presence of a gastric submucosal tumor 4 cm in diameter in the lesser curvature of the gastric body. Laparoscopic partial gastrectomy was performed by inserting three ports into a single incision made in the umbilicus. Post-operative course was uneventful. Pathological diagnosis was a low-risk GIST. Single-incision laparoscopic partial gastrectomy has the potential to become one option for low-invasive therapy for gastric GIST.
