腹腔鏡下に切除し得た異物穿通による大網炎症性腫瘤の1例

Successful laparoscopic assisted resection of foreign body granuloma in the greater omentum diagnosed preoperatively by 3D-CT

　症例は61歳の女性．左下腹部のしこりを自覚したため他院を受診し，腹部CT検査で腹腔内腫瘤を指摘され，精査・加療目的で当院を受診した．腹部CTで大網と思われる脂肪濃度の中に線状の石灰化像を認めた．3D-CTでは石灰化像の辺縁はさらに明瞭となり，腫瘍による石灰化よりも魚骨などの異物を強く疑わせた．異物誤嚥の自覚はなかったが，魚骨の穿通による大網炎症性肉芽腫と診断し，を施行した，腹腔内を検索したところ，大網に腫瘤を認め，腹壁と強固に癒着していたが周囲の腸管とは癒着しておらず，剥離を終えた後に左下腹部に小切開を加え，腫瘤を摘出した．腫瘤の中に小さな膿瘍腔を認め，内部には魚骨と思われる異物が存在しており，魚骨穿通による慢性炎症性肉芽腫と診断された．腹腔鏡下手術は魚骨などの異物穿通による慢性型の炎症性肉芽腫に対しても非常に有効な治療選択肢の1つとなり得ると思われた．

We report a case of successful laparoscopic assisted resection of foreign body granuloma diagnosed preopera-tively by 3D-CT. A 61-year-old woman was admitted because of left lower abdominal mass. Computed tomography (CT) revealed a 1.5 cm linear shadow in the mass lesion. Furthermore, 3D-CT showed this linear shadow more clearly. We thus diagnosed an intraabdominal foreign body granuloma due to a fishbone, and per-formed laparoscopic assisted surgery. The mass was located in the greater omentum, and adhered to the ab-dominal wall strongly, but not adhered to the intestine.
