◆要旨：患者は60歳代，女性．下血を主訴に当院を受診した．上部消化管内視鏡検査，下部消化管内視鏡検査を施行したが，出血源は不明であった．出血シンチグラフィー検査，血管造影検査にて下部小腸からの出血と診断した．重度の貧血があり血圧の維持が困難であったために緊急手術を施行した．術中腸管外から出血部位は確認できなかった．腹腔鏡にて腹腔内を観察し，適切な位置かつ最小限の開腹創から小腸を創外へ取り出した．腸管に小切開を加えて内視鏡を挿入し，病変部位を確認したうえでクリップにて止血を行った．腹腔鏡下手術に併用した内視鏡が小腸出血に対する治療の選択肢の1つとして考えられた．
A 69-year-old woman was admitted to our hospital with melena. The upper and lower gastrointestinal endoscopy showed no abnormal findings. Scintigraphy showed an accumulation in the right lower quadrant. Angiography revealed bleeding from a peripheral region of the final branch of the ileal artery into the ileal lumen. Emergency operation was conducted. Intraoperative endoscopy identified the bleeding point and the clip was used to stop the hemorrhage. Appropriate incision site and minimum wound size were possible by the use of laparoscope. Laparoscopic surgery with the endoscope may be considered as one of the choices for the operative treatment of small intestines hemorrhages.
