下腿外側（前方・外側）筋コンパートメントからのSEPS

Subfascial endoscopic perforator surgery (SEPS) for lateral venous ulcer and stasis syndrome

　今日，下腿内側にうっ滞性潰瘍や皮膚硬化を持つ下肢静脈瘤症例に対し内視鏡下筋膜下不全穿通枝切離術（subfacial endoscopic perforator surgery：SEPS）は標準術式となってきている．下腿外側，外果周囲に皮膚病変を持つ症例にも不全穿通枝（incompetent perforating venis：IPV）切離は重要な治療手段であり，その際には健常皮膚から内視鏡下アプローチによるSEPSが低侵襲で有利である．下腿外側でのSEPSは，筋膜下操作は，内側SEPSと同様である．しかし下腿外側では，筋膜下手術操作腔が狭い，IPVがしばしば筋肉間，つまり筋膜外に存在するなどの異なる点も存在するため，本稿において下腿外側SEPSの手技の実際とポイントについて述べた．

Today, subfascial endoscopic perforator surgery (SEPS) on the medial side of lower leg has become popular in the treatment of a stasis ulcer. However, laterally located venous ulcer are of equal importance. In this report, the essential points of lateral SEPS and our procedure of lateral SEPS were shown. In our report, we used the one port system (OLYMPUS). SEPS by the one port system is advantageous in terms of securing a good opera-tive field, because the lateral subfascial space is narrow.
