単腎症例に発生した腎細胞癌に対し腎動脈遮断による後腹膜鏡下腎部分切除術を施行した1例

Retroperitoneoscopic partial nephrectomy with renal artery clamping for renal cell carcinoma in a solitary kidney

　患者は56歳，男性．2002年に左腎細胞癌に対し開腹にて根治的腎摘除術を施行した．その時点で右腎に径3cmの複雑性嚢胞を認めていた．経過観察中の2003年のCTにて嚢胞壁の造影効果が増強したので，嚢胞性腎細胞癌の可能性が否定できなくなり，2004年1月に腎動脈遮断による後腹膜鏡下腎部分切除術を施行した．手術時間128分，出血量100ml，温阻血時間40分であった．術前・術後の血清クレアチニンはそれぞれ1.4mg／dl,1.5mg／dlと術後の腎機能障害を認めなかった．病理組織学的診断は嚢胞随伴性腎細胞癌であった．本術式は単腎症例でも選択肢の1つになりうると考えられた．

A 56-year-old man. who had undergone open radical nephrectomy for lt renal cell carcinoma in 2002, had a 3 cm complicated cyst in his rt kidney. During the follow-up, CT revealed that the septum of the cyst became more markedly enhanced in 2003. Under the diagnosis of suspected renal cell carcinoma, the patient underwent retroperitoneoscopic partial nephrectomy with renal artery clamping on January 2004. Operative time was 128 minutes and estimated blood loss was 100 ml. Ischemia time was 40 minutes.
