胃粘膜下腫瘍像を呈した肝血管腫の1例

Hepatic hemangioma presented an image of a gastric submucosal tumor:A case report

　肝血管腫はまれな腫瘍ではないが，肝外に発育し胃粘膜下腫瘍像を呈した症例を経験した．症例は52歳，女性．検診で胃穹窿部に異常を指摘され，当院にて受診した．胃透視，胃内視鏡および超音波内視鏡検査にて穹窪部大彎側に3cm大の粘膜下腫瘍が疑われた．CT検査でも同様に3cm大の腫瘤像を認めた．これらの所見から胃のGISTなどの粘膜下腫瘍を疑い手術を施行した．腹腔鏡下に観察すると肝外側区域から突出した4cm大の肝血管腫を認め，これが胃を圧迫し粘膜下腫瘍像を呈していたものと判断し，肝部分切除をした．近年，胃粘膜下腫瘍に対し腹腔鏡下に胃部分切除をする機会はまれではない．術前鑑別診断として肝外発育性肝血管腫も念頭に置くべきと思われる．

Hepatic hemangioma is not a rare type of tumor, but we recently experienced a tumor that developed extrahepatically and presented an image of a gastric submucosal tumor. The patient was a 52 year-old woman. She visited the hospital following a medical check-up that detected an abnormality in the gastric fundus. As a result of gastric fluoroscopy, gastroscopy and endoscopic ultrasonography, a 3 cm submucosal tumor was sus-pected on the greater curvature of the gastric fundus. CT examination also confirmed an image of a 3 cm tumor.
