　脳室—腹腔シャント（ventriculo-peritoneal shunt：VPS）を有する高齢早期胃癌患者に対して，腹腔鏡補助下幽門側胃切除術（LADG）を施行した．術後合併症に伴う逆行性感染を防ぐため腹腔鏡下にVPSの腹腔側カテーテルをいったん抜去した．第7病日の頭部CTで水頭症の進行を認め，傾眠傾向が出現したため第26病日に腹腔鏡補助下VPS再造設術を施行した．腹腔鏡は腹腔内の癒着部位の把握，カテーテルの刺入部位や先端位置の決定，先端からの脳脊髄液流出の確認に有用であった．腹腔内の癒着を減らしVPSに伴う合併症を防ぐためにも，VPSを有する患者には腹腔鏡下手術を可能な限り行うべきであると思われた．

An 87 year-old female with ventriculo-peritoneal shunt (VPS) underwent laparoscopy-assisted distal gastrectomy (LADG). The peritoneal side of the VPS catheter was ligated, divided and pulled out before pneumoperitoneum. At 7 post-operative days, follow-up brain computed tomography was performed and it showed progression of hydrocephalus. Laparoscopy-assisted distal VPS placement was performed on the 26 post-operative day. It showed adhesion in the peritoneum.
