脳室腹腔シャント留置患者に対する腹腔鏡下胆囊摘出術の経験

Laparoscopic cholecystectomy in a patient with a ventriculoperitoneal shunt

◆要旨：Ventoriculo-peritoneal shunt（以下，VPS）留置患者に対して腹腔鏡下胆囊摘出術を施行した．逆流防止弁を装備したシャントシステムであり，術前X線透視にてシャントチューブの走行を確認した後，チューブをクランプしたり，他の処置をすることなく手術を行った．CO₂気腹法を行い，最大気腹圧を10mmHgとした．術後明らかな頭蓋内圧亢進徴候，チューブトラブルや感染はなく経過良好であった．逆流防止弁のあるVPSが留置された患者に対して，チューブの処置下に腹腔鏡下胆囊摘出術が安全に施行されうると考えられた．

　A 58 year-old man with a one-way ventoriculo-peritoneal shunt（VPS）was admitted to our hospital under the diagnosis of cholelithiasis. Laparoscopic cholecystectomy（LC）was carried out without clamping or other manipulations of the shunt after the examination of the intra-abdominal route of the shunt by X-ray. The maximun CO2 insufflation pressure was set at 10 mmHg. He recovered well postoperatively with neither signs of increased intracranial pressure nor any signs of shunt failure or infection. This case illustrates that LC could be done safely in patients with a one-way valve VPS without any treatment of the shunt.
