脳室腹腔シャント術腹腔側シャントチューブ留置時における腹腔鏡補助下カテーテルイントロデューサーⓇ 使用の有用性

Effectiveness of CatheterintroducerⓇ in laparoscopy-assisted placement of the abdominal ventriculo-peritoneal shunt catheter

◆要旨：脳室腹腔シャント術（ventriculo-peritoneal shunt：以下，VPシャント術）は水頭症に対し最も一般的に行われている治療である．近年，シャント機能不全を予防するために2ポートによる腹腔鏡下VPシャント術が散見され，その有用性が報告されている．今回，筆者らは新しい工夫として腹腔鏡手術の利点である整容性をさらに追及するとともに，より簡便に腹腔側シャントチューブを目的部位に留置できるようカテーテルイントロデューサー^Ⓡを使用した．カテーテルイントロデューサー^Ⓡは挿入器具としてだけではなく，鉗子の役割まで果たすため，カメラポート1ポートのみでの手術が可能となり，手術時間も短縮された．

　The ventriculo-peritoneal（VP）shunt operation is the standard therapy for hydrocephalus. Recently some reports have shown effectiveness of laparoscopic VP shunt using 2 ports with the goal of avoiding shunt failures. In order to achieve a minimum incision and a more simple method, the shunt catheter was inserted using a Catheterintroducer^Ⓡ. Because the Catheterintroducer^Ⓡ has both the insertion item and the forceps, it enables the surgeons to perform the operation quickly and with only one port.
