腹腔鏡下虫垂切除術にて摘出しえた虫垂内異物（待ち針）の1例

Successful laparoscopic appendectomy for a foreign body (marking pin) in the appendix. Report of a case

　患者は39歳，女性，待ち針を誤飲し来院した．腹部単純X線検査ではすでに胃内に落下しており，自覚症状がないことから経過観察となった．誤飲から2日後，右下腹部より針の移動がなく，誤飲から71日後，自然排出する可能性が低いと考えられたため腹腔鏡下に摘出手術を行った．術中X線透視を併用し腹腔内を検索すると，異物は虫垂内に存在し安全に虫垂切除にて摘出しえた．病理所見は，金属アレルギー反応に特徴的な好酸球主体の蜂窩織性虫垂炎であった．虫垂内異物（針）はきわめてまれであり，鋭利な金属性異物を腹腔鏡下に摘出する場合，術中X線透視が有用であり，かつ通常より愛護的操作が必要である．

A 39-old-year female was admitted to our hospital because she swallowed a marking pin accidentally. She had no abdominal pain and was treated conservatively. The marking pin was fixed in the right lower quadrant of the abdomen on the second day of accidental swallowing. We suspected that this foreign body would not be discharged spontaneously, so, laparoscopic surgery using X-ray image scope was performed on the seventy-first day of accidental swallowing.
