子宮外妊娠：卵管保存手術

Laparoscopic conservative surgery for ectopic pregnancy

　子宮外妊娠に対する妊孕能温存手術としては，腹腔鏡下線状切開（laparoscopic salpingotomy）が標準術式として挙げられる．その適応基準としては，挙児希望例が絶対条件であり，その他，卵管の部位や大きさ，術前の血中hCG値，破裂あるいは胎児心拍の有無などにより選択されるが，必ずしも統一した基準が示されているわけではない．保存手術が成功した場合に，卵管疎通率は80％程度，術後妊娠も60〜70％程度と報告されており，妊孕能保存の手術として試みてよい術式と思われる．しかし，外妊存続症あるいは反復外妊といった副障害もみられることから，患者や家族への十分なインフォームド・コンセントは必須である．

Nowadays laparoscopic salpingotomy has become the gold-standard for the treatment of patients with ectopic pregnancy who wish to be pregnant in the future. However, the indication and limitation of laparoscopic salpingotomy have not been firmly established, especially regarding the location and the size of the tube, the level of preoperative serum hCG, ruptured or unruptured pregnancy sac, and patients with positive fetal heart beat.
