術中ENBDチューブ造影が有用であった腹腔鏡下遺残胆嚢摘出術の2例

Laparoscopic removal of gallbladder remnant with intraoperative cholangiography via endoscopic nasobiliary drainage tube:report of two cases

　胆嚢摘出術後の遺残胆嚢および胆嚢管内に存在する結石に対し，ときに根治手術が必要となる症例を経験するが，その多くは開腹手術が選択される．今回，われわれは開腹胆嚢摘出術後の遺残胆嚢・胆嚢管内胆石症に対し，内視鏡的経鼻胆道ドレナージチューブ（ENBDT）を利用して術中胆道造影を行って腹腔鏡下に切除した2例を経験した．2例とも術前日にENBDTを留置した．ともに高度の癒着を認め，2例目では胆嚢管と総胆管を誤認したが，術中ENBDT造影で胆管損傷を回避でき腹腔鏡下に手術を完遂できた．遺残胆V・胆V管内胆石症のなかにも低侵襲な腹腔鏡下手術で切除しうる症例が存在するが，手術に際しては術中ENBDT造影が有用であり，必須と考えられた．

We often experience cases involving a gallbladder remnant or cystic duct remnant with calculi following a previous partial cholecystectomy where the symptoms require a radical operation. In many instances laparotomy is chosen. We encountered two cases where ongoing cholelithiasis deposited calculi in gallbladder/ cystic duct remnants. We treated these patients by laparoscopic surgery performed with intraoperative cholangiography using an endoscopic nasobiliary drainage tube (ENBDT).
