早期胃癌のリンパ節転移と郭清範囲—内視鏡外科の立場から

Nodal metastasis in early gastric cancer and target of laparoscopic dissection

　早期胃癌に対する腹腔鏡補助下縮小手術は「胃癌治療ガイドライン」では「臨床研究」として評価されるべきものとして取り上げられている．リンパ節の転移状況と腹腔鏡の特性を考えた郭清範囲を検討した．早期胃癌のリンパ節転移頻度は低く，転移がほぼない条件も設定できるが，2群転移も3％程度認めた．また，2群郭清効果は良好である．積極的に腹腔鏡下手術を導入している施設では，腹腔鏡補助下幽門側胃切除（縮小手術）の手術手技は確立されている．しかしながらD2郭清，全摘，噴門側胃切除はその域でない．患者の利益を考えると，現状では幽門側胃切除のD1＋α，βが推奨できる郭清範囲と考える．

According to “Gastric Cancer Treatment Guidelines” by the Japanese Gastric Cancer Association, laparo-scopic modified gastrectomy for early gastric cancer is still considered as investigational treatment. In this study, characteristics of nodal status in early gastric cancer and appropriate extent of node dissection for laparoscopic gastrectomy were investigated. Although the incidence of nodal metastasis was very low, approxi-mately 3% of the patients had N2 node metastasis.
