早期胃癌に対するsentinel node navigation surgery

Sentinel node navigation surgery for early gastric cancer

　腫瘍から直接リンパ流を受ける見張りリンパ節（sentinel node：SN）を指標とした早期胃癌のリンパ節転移診断に関して，その有用性を示唆する報告が相次いでいる．SNの分布と転移の有無を指標としてリンパ節郭清を個別的に縮小ないし省略し，それに伴って切除範囲を最小限とするsentinel node navigation surgery（SNNS）を腹腔鏡下に施行することにより，リンパ節転移陰性早期胃癌の外科治療のさらなる個別化，低侵襲化が期待される．現時点では多くの技術的課題が残されており，現在進行中の開腹手術における二つの胃癌SN生検多施設共同研究の結果に基づいて，腹腔鏡下におけるSN生検標準手技を確立することが期待される．

Sentinel nodes (SN) are defined as directly draining nodes from the primary lesion and representatives of the regional lymph nodes in terms of metastatic status. Recently several reports demonstrated the validity of this concept in early gastric cancer in open surgery. The laparoscopic application of this technology is one of the top-ics in gastric cancer surgery.
