胸腔鏡下に根治切除術が可能であった肝硬変合併胸部食道癌の1例

Successful thoracoscopic esophagectomy in a patient with esophageal cancer and liver cirrhosis

　胸腔鏡下根治切除術が可能であった肝硬変合併食道癌症例を経験した．患者は64歳男性．切歯から20cmの部位に1pl型の隆起性病変を認め，生検で低分化型扁平上皮癌と診断された．食道静脈瘤はなかった．CT上で肝硬変像を呈しており，ICG15分値は23.8％で肝障害度Bであった．2002年5月8日に胸腔鏡下食道切除術を行った．下部食道周囲の静脈に怒張を認めたが，クリッピングで止血が可能であった．上縦隔徹底郭清のため，気管分枝部から頭側の胸管を合併切除した．胸腔内操作時間は215分，胸腔内出血量は110gで輸血は行わなかった．病理組織学的進行度はpT1b pN0 pIM0 inf β ie（−） v0 ly0 stage Iであった．術後はAST・ALTが一時的に軽度の上昇を認めたが合併症はなく経過し，術後30日目に退院した．肝硬変合併食道癌症例でも食道静脈瘤を伴わない症例には胸腔鏡下食道切除術は可能であり，手術侵襲の軽減によって術後合併症のリスクを低下させ得ることができた．

We successfully treated a patient with thoracic esophageal cancer and liver cirrhosis by thoracoscopic esoph-agectomy. A 64-year-old man had an elevated (1pl type) lesion 20 cm from the incisors identified by endoscopic examination. Histology of the biopsy specimen was poorly differentiated squamous cell cancer. There were no esophageal varices. Cirrhosis of the liver was diagnosed by abdominal CT, and the ICG 15 minutes was 23.8%, indicating grade B liver damage.
