胸部食道癌に対する胸腔鏡下食道切除術・リンパ節郭清および後縦隔胃再建後の乳び胸に対して，食道切除時に用いた小開胸およびポート挿入部から胸腔鏡下胸管結紮術を行った．フイブリンの付着を認めたが，臓側および腹側胸膜には切離を要する癒着はなかった．肺の適切な圧排によって，横隔膜頭側の椎体前面と大動脈右側面の展開は容易であった．椎体前面の臓側胸膜に2cmの縦切開をおき，下層を縦走する胸管を露出，同定して二重結紮した．結紮後，乳び漏出は停止した．胸腔鏡下胸管結紮術は低侵襲で有効な術式である．
Thoracoscopic ligation of the thoracic duct for chylothorax after throacoscopic radical esophagectomy for can-cer was performed. The mediastinum was accessed through mini-thoracotomy and port sites used for the e-sophagectomy. Despite sticking of fibrin deposit on the parietal and visceral pleura, no adhesion between these pleura requiring dissection was encountered. The dorsal aspect of the vertebra and the right aspect of the aorta were easily exposed with efficient retraction of the right lung just cranial to the diaphragm.
