横隔膜弛緩症に対して胸腔鏡下横隔膜縫縮術を施行した1例

VATS plication of diaphragmatic eventraion

　横隔膜弛緩症に対して胸腔鏡下横隔膜縫縮術を施行した1例を経験した．患者は74歳，女性．自覚症状は右側臥位時の胸痛であった．胸部X線写真では横隔膜ヘルニアとの鑑別は困難であったが，胸部MRIで確定診断ができた．呼吸機能は正常で自覚症状も重篤なものはなかったが，患者の手術の希望も強かったため，低侵襲の胸腔鏡下横隔膜縫縮術を施行した．胸腔操作の施行前に腹腔鏡を挿入し，腹腔内臓器と横隔膜の癒着を検索した．大網との癒着を軽度に認め，これを？離したのち胸腔操作で安全に縫縮術を施行し得た．術後，胸痛は消失し，X線上も横隔膜の下降を認めた．現在まで再発やヘルニアは認めていない．

We reported a case of video-assisted thoracoscopic surgery (VATS) plication for diaphragmatic eventraion. A 74-year-old woman complained of a pain in her left chest in the right lateral position. It was difficult to distin-guish diaphragmatic eventration from diaphragmatic hernia by chest X ray. Chest MRI revealed elevation of the diaphragm. Respiratory function was normal and her symptom was not severe, but she requested surgery and VATS plication was performed.
