小開胸創に用いる新しい開創器ラッププロテクターHを用いた小開胸を併用した胸腔鏡下手術

VATS Lobectomy with Minithoracotomy with a Newly Invented Ecarteur (Lapprotector H)

　胸腔鏡下手術で肺葉切除（VATSlobectomy）やリンパ節郭清を行うときには小開欄胸創が併用されるが，術後の疼痛を最小限にするためには小開胸創に従来の金属製の小開胸器は使用しないのが望ましい．そこで，われわれは小開胸創に用いる新しい開創器ラッププロテクターHを開発した．これは従来の開胸器を使用することなく適当な大きさの小開胸創の創口を確保し，軟部組織を圧排でき，しかも手術器械の操作の妨げとならず，小開胸創の創縁も保護することができる．このラッププロテクターHを使用することで，VATSlobectomyやリンパ節郭清をする際に開胸器を使用しなくても小開胸創を有効に使うことができ，肺癌に対する胸腔鏡下手術の安全性と確実性を高め，開胸手術に近い容易な手術手技を可能にすると考えられる．また，従来の開胸器を使用したVATSlobectomy症例に比較して術後疼痛を軽減できる可能性が示唆された．

Video-assisted thoracic surgery (VATS) has been applied to clinical Stage IA lung cancer. Usually we per-form lobectomy and lymph-node dissection by VATS with minithoracotomy. In this operation a rib retractor has been used to spread the intercostal space for the minithoracotomy, but it may cause postoperative pain. So we newly invented an ecarteur of the chest wall (Lapprotector H). This ecarteur is very useful to get an ade-quate intercostal space and retract the soft tissue pad.
