精巣腫瘍に対する腹腔鏡下後腹膜リンパ節郭清術

Laparoscopic Retroperitoneal Lymph Node Dissection for Testicular Cancer

　より侵襲の少ない治療法を目指して，泌尿器科領域でも腹腔鏡下手術が発達し，その適用範囲が拡大している．精巣腫瘍の後腹膜リンパ節転移に対する腹腔鏡下後腹膜リンパ節郭清術は，1992年にHulbertらによって初めて報告されて以来，外国では90例ほど実施されているが，本邦での報告例はない．今回われわれは4例の腹腔鏡下後腹膜リンパ節郭清術を試みたので，自験例の成績，手術手技，腹腔鏡下後腹膜リンパ節郭清術の有効性について報告した．

Laparoscopic surgery is widely accepted in the urological field as a minimally invasive treatmentmodality, and its indication has been extended. The laparoscopic procedure was applied first to retroper-itoneal lymphnode dissection (RPLND) for metastatic testicular tumor by Hulbert in 1992. The cases oflaparoscopic RPLND have been complied to about 90 so far in western countries, but there is no report fromJapan. Recently, we experienced four cases with testicular cancer who underwent laparoscopic RPLND. Theresults, technique and significance of laparoscopic RPLND are discussed.
