十二指腸球部前壁カルチノイドに対し内視鏡補助下に腹腔鏡下十二指腸部分切除を施行した1例

A Case Report of a Duodenal Carcinoid Treated with Endoscopy-assisted Laparoscopic Partial Resection

　十二指腸球部前壁カルチノイドに対し，内視鏡補助下腹腔鏡下十二指腸球部前壁楔状切除を施行し得た症例を経験した．症例は82歳，女性．心窩部痛を訴えて来院した．上部消化管内視鏡検査で十二指腸球部前壁に径5mm大のカルチノイドを認め入院した．手術は内視鏡補助下に腹腔鏡下球部前壁楔状切除を施行した．病理所見では粘膜下層までにとどまるカルチノイドで，切除断端は陰性であった．術後2年経過した現在も再発兆候は認めない．本症例のような前壁側に限局した腫瘍では，内視鏡補助下腹腔鏡下十二指腸楔状切除は低侵襲，根治性を両立し得た治療法であると考えられた．

We report a case of Duodenal Carcinoid which was treated successfully with endoscopy-assisted laparoscopic wedge resection of the duodenal anterior wall. An 82-year-old woman was referred to our hospital with epigastralgia. She had a duodenal carcinoid at the anterior wall of the bulbus.which was diagnosed by a biopsy under endoscopy. The tumor was removed by endoscopy-assisted laparoscopic wedge resection of the duodenal anterior wall. The histology confirmed the carcinoid was confined to the submucosal layer, and the resected edge was tumor free. Two years after the operation.
