腹腔鏡下神経鞘腫摘出術の1例

A Case of Resection of Spinal Schwannoma Performed under Laparoscopy

　腹腔鏡下に脊髄神経鞘腫摘出を行った1例を経験した．症例は54歳，女性．主訴は歩行時の左下肢ひきつれ感出現と尿失禁である．筋力，知覚，深部腱反射の異常はなかったが，左大腿筋萎縮と膀胱直腸障害があった．MRIで腫瘍はT12／L1レベルの脊柱管内から後腹膜腔に広がり，砂時計型を呈していた．腹腔鏡下に腫瘍の可及的摘出を行い，腫瘍はT12／L1レベルの交感神経に発生したものと考えられた．病理所見はAntoni A typeのSchwannomaであった．術後創部痛はほとんどなく5日で退院となり，症状は消失した．可及的摘出で術後早期に神経症状が改善したことから，腫瘍自体の脊髄牽引作用があったものと推察された．

We present a case of successful treatment of spinal schwannoma performed under laparoscopy. The case is a 52-year-old woman who complained of paresthesia of the left lower extremity and incontinence of urine. There were no neurological deficits. MRI showed a paravertebral dumbbell tumor sized 4×6 cm at the T12/ L1 level, which slightly invaded the spinal canal. Subtotal resection was performed only from the anterior approach under laparoscopy. Because of no continuity of spinal nerve root at T12, we diagnosed the tumor originated from the sympathetic nerve.
