転移性肺腫瘍に対する胸腔鏡的アプローチ

Video-assisted Thoracoscopic Surgery for Metastatic Lung Tumors

　15歳以下の小児転移性肺腫瘍に対する胸腔鏡下肺手術の有用性，問題点を検討し，報告した．12歳，14歳の骨肉腫転移性肺腫瘍2症例に計4回の胸腔鏡下肺切除を施行した．側臥位，分離換気下に第4〜7肋間に5〜10mmのポートを逆三角形になるよう挿入し，Endo GIA3〜4本で肺部分切除を施行した．この術式による血気胸などの合併症はみられず，4回の平均手術時間は96分であった．病変部の同定が困難な場合に小開胸を追加した．胸腔鏡下肺切除は手術侵襲が少なく，同側再手術時も胸腔内の癒着をほとんど認めなかった．乳幼児例に対する分離換気の工夫，術中の小転移巣の同定に術前CTガイド下マーキングなどの工夫が必要と思われた．最近，2〜3mmの硬性鏡，操作用鉗子が開発され，今後，小児例に対する胸腔鏡下手術が普及するものと思われる．

Video-assisted thoracoscopic surgery (VATS) has been recently used in diagnostic and therapeutic procedures such as lung biopsy, resection of bulla. etc. However, there have been few reports of its use in performing wedge resection for metastatic tumors. We have operated on two patients under the age of 15 years who had metastatic lung tumors from osteosarcoma by means of VATS since July 1991. Wedge resection of pulmonary nodules could be safely accomplished with the Endo GIA linear cutting stapler without any morbidity.
