 Various complications can be caused by transcatheter arterial chemoembolization(TACE) for the treatment of hepatocellular carcinoma(HCC). Supraumbilical skin injury occurs by the flow of chemotherapeutic agent into the hepatic falciform artery (HFA), but this artery has received little attention by angiographic investigators. We report a case of HFA, a 73-year-old female suffering from HCC. We identified the HFA arising from middle hepatic artery on arteriography prior to chemoembolization, but it was difficult to insert the catheter selectively and occlude it. To avoid supraumbilical skin injury, laparoscopic ligation of round ligament and HFA was performed prior to transcatheter arterial chemoembolization. We should carefully perform TACE for HCC in the left lobe.
