腹腔鏡下に摘出した重複胆囊の1例

A case of gallbladder duplication treated by laparoscopy

◆要旨：患者は49歳，男性で，健診目的に受けた腹部超音波検査にて胆囊の異常を指摘された．CT，MRCPにて重複胆囊と診断し，腹腔鏡下に両胆囊を摘出した．Boyden分類H型，Gross分類B型の重複胆囊で，病理学的には一方の胆囊に慢性炎症の所見がみられた．術後は合併症なく経過した．

　重複胆囊は稀な胆道系の形態異常であり，手術の際は術前に正確な分類診断をすることが不用意な胆道系損傷を避けるために重要であると思われた．

　A 49-year-old man presented with an abnormal gallbladder as evaluated by US during a health checkup and was later diagnosed as having double gallbladders on preoperative CT and MRI. He was then successfully treated by laparoscopic cholecystectomy. This case was H-shaped type by Boyden's classification and type B by Gross's classification. Histopathologically, chronic inflammation was seen in one of the gallbladders. The postoperative course was uneventful.

　Gallbladder duplication is a rare anatomic malformation. Accurate preoperative differentiation of the type of gallbladder duplication is required to avoid inadvertent damage to the biliary duct system.
