◆要旨：重複胆囊は，診断に難渋することがある非常に稀な解剖学的形態異常である．われわれは，急性胆囊炎を発症した重複胆囊症例に対して腹腔鏡下胆囊摘出術を施行した1例を経験したので報告する．急性胆囊炎による炎症所見が高度であり，術前に重複胆囊の診断をすることはできなかった．また，術前画像検査では指摘されなかった重複胆囊管が術中胆道造影により明らかになった．術中，術後とも合併症なく経過した．重複胆囊に対して外科治療を行う際は，重複胆囊管などその他の胆道系の形態異常を合併する可能性があるので，胆道損傷に十分に注意して施行するべきである．
Multiple gallbladders are a rare congenital malformation and difficult to daiagnose. We present a case of multiple gallbladders with acute cholecystitis that was successfully managed by laparoscopic cholecystectomy. Preoperative diagnosis was difficult due to inflammation caused by acute cholecystitis. Intraoperative cholangiography revealed double cystic ducts that were not shown on preoperative imaging studies. Multiple gallbladders may be associated with other malformations of the biliary tract, and surgical operations should be performed with extreme caution in order to avoid bile duct injuries.
