「内視鏡外科診療ガイドライン」出版はどのように実臨床に影響しているか？―アンケート調査結果から―

How does the first publication of a monograph entitled“Practice Guidelines for Endoscopic Surgeries”influence on general clinical practice? : based on the results of nation-wide questionnaire survey

◆要旨：「内視鏡外科診療ガイドライン（以下，診療GL）」の発刊が臨床現場にどのように影響しているかを検証することは内視鏡外科の今後の発展のために重要な課題である．今回の調査は一般消化器外科，小児外科，整形外科に限定して行い，アンケート用紙の全送付数2,111中の34％から回答を得た．アンケートの質問は14領域共通項目と各領域に特異な複数項目で構成した．“診療GLを常にあるいは時折参照”など診療GLの有用性に賛同する回答は三学会に共通しており，55～75％であり，診療GL出版後の治療方針の変更については20％に認められている．しかし，低い回答率が示すように，診療GLの実地臨床への浸透は必ずしも十分といえない．新たに蓄積されるエビデンスを積極的に組み入れて，数年単位で改訂作業が繰り返されることが本診療GLの意義を高めるものと思われる．

　It would be important to answer the question“how does the publication of a monograph entitled Practice Guidelines for Endoscopic Surgeries（Guidelines）influence on general clinical practice?”for further development of endoscopic surgeries in our nation. The current survey was performed for surgeons who work just for general, gastroenterological, pediatric and orthopedic fields by sending questionnaires in nation-wide. Thirty-four percent of surgeons（709/2111）responded to this survey, 50?75％of the responders recognize the usefulness of the Guidelines and 20％have changed their treatment methods based on the Guidelines. However, as indicated by the lower response rate to this survey, the influence of the Guidelines on current clinical practices seems insufficient. Its repetitive revisions, that include newly accumulating evidence, in several years' term will be necessary for increase of the significance of this Guidelines.
