わが国における内視鏡外科修練に関する全国調査報告—日本内視鏡外科学会卒後10年目以下会員対象アンケート調査

A nationwide survey on endoscopic surgery training in Japan

◆要旨：【背景と目的】わが国の内視鏡外科領域の修練状況を把握する目的で，本邦初の全国調査を実施した．【方法】卒後10年目以下の日本内視鏡外科学会会員を対象とした．アンケートは内視鏡外科トレーニング環境，腹腔鏡下手術8術式に対する執刀経験数および自己評価による習熟度を問う内容とした．【結果】アンケート調査の回答率は28.1％であった．多くの回答者が腹腔鏡下虫垂術および胆囊摘出術の執刀に自信をもっている一方で，腹腔鏡下大腸切除および胃切除については習熟度に自信がないという回答が多くを占めた．【結語】本調査結果が，わが国のより良い外科修練システム構築のために有用な情報として活用されることを期待する．

　［Purpose］ There is no standard curriculum for endoscopic surgery training in Japan, and competency in these procedures has not been investigated by nationwide survey. The aim of this survey was to research the current status of endoscopic surgery training in Japan. ［Methods］ A questionnaire survey was mailed to 2,296 members of the Japan Society for Endoscopic Surgery who were between postgraduate year 3 and 10. The questionnaire inquired about endoscopic surgical training conditions, operation case numbers, and autonomy in 8 laparoscopic procedures. ［Results］ The total response rate was 28.1％. The number of cases required to perform procedures independently were demonstrated. Most participants felt confident in performing laparoscopic appendectomy and cholecystectomy; however, felt less confidence about more advanced procedures such as laparoscopic colectomy and gastrectomy. ［Conclusions］ The information from this survey may be useful for surgical educators, surgical societies, and the board certification council to consider the future surgical training system in Japan.
