先天性気管支閉鎖症に対する完全胸腔鏡下左上大区域切除術の1例

A case of congenital bronchial atresia treated complete thoracoscopic left upper division segmentectomy

◆要旨：先天性気管支閉鎖症に対して，完全胸腔鏡下区域切除術を施行した．患者は16歳，女性．胸部CTで左上大区に囊胞性病変と粘液貯留を認めた．気管支鏡検査とmulti-detector row computed tomography（以下，MDCT）にて上大区気管支入口部欠損を認め，気管支閉鎖症と診断した．完全胸腔鏡下に左上大区域切除を行い，合併症はなく手術を終了した．第1病日目に胸腔ドレーンを抜去し，第4病日目に退院した．良性疾患である本疾患は，整容性に優れた完全胸腔鏡下手術のよい適応と考えられた．

　We reported a case, a 16-year-old woman of congenital bronchial atresia（CBA）treated by thoracoscopic pulmonary segmentectomy. Cystic lesions and bronchial mucocele on the left upper division was detected in chest CT. We found a defect of the bronchial orifice of the left upper division under bronchoscope as well as multi-detector row CT（MDCT）findings. Under these findings she was diagnosed as CBA. Thoracoscopic left upper division segmentectomy was performed successfully, and post-operative course was uneventful with minimal invasiveness. Chest drain was removed on 1 post-operative day and she was discharged 4 days after the operation. Thoracoscopic segmentectomy was considered to be a cosmetically feasible and minimally invasive method for the treatment of benign pulmonary lesions such as CBA.
