◆要旨:HIFUとはhigh intensity focused ultrasoundの略で,別名FUS(focused ultrasound surgery)とも呼ばれるが,超音波を焦点に収束させ,そこで発生する熱を利用して組織を熱凝固させる技術であり,良性,悪性腫瘍の治療に用いられている.超音波やMRIを併用して,術中に病変の変化をリアルタイムでモニターしながら治療が可能であり,非侵襲的ながんの治療法として注目を集めている.肝癌や腎癌,膵癌,乳癌,骨肉腫や骨転移などの固形腫瘍に応用され,また,子宮筋腫などの良性腫瘍の治療にも使われる.前立腺癌に経直腸的に使われる小型のHIFUもあり,これらを改良すれば,内視鏡外科手術にHIFUを併用することも可能になると期待される.
HIFU(high intensity focused ultrasound)or FUS(focused ultrasound surgery)is a device that uses focused ultrasound to make a heat and coagulate tissues to treat benign or malignant tumors non-invasively. Combined with ultrasound or MRI monitoring, the tumors are observed real-time during the HIFU procedure. HIFU treatment is effective for malignant tumors such as liver, kidney, pancreas, breast and bone tumors. Bone metastases and uterine myoma are also good candidates for HIFU treatment. Smaller HIFU is now used to treat prostatic carcinoma transrectally, and we can expect that HIFU will be utilized with endoscopic surgery in the near future.
