片側性多嚢胞性異形成腎に対する腹腔鏡下腎摘出術

Laparoscopic Nephrectomy for Infant with Unilateral Multicystic Dysplastic kidney

　小児の腹腔鏡下腎摘出術について，8か月の女児の片側性多嚢胞性異形成腎の症例を中心に，その手技について述べた．片側性多嚢胞性異形成腎に対する摘出の是非については賛否両論の意見がみられるが，本術式の導入により，開腹術に比べ侵襲も少なく安全であることから，今後，小児のほかの泌尿器疾患と同様に適応の拡大が予想される．しかし，小児では適応となる疾患や症例の絶対数が少ないことからlearning curveが得られにくく，個々の症例において十分な検討と綿密な工夫が必要となるうえに，適応の決定には慎重でなければならない．そして，時と場合によっては決して無理をせず，速やかに開腹術に変更する勇気ある決断に躊躇すべきでない．

Laparoscopic treatment is now gradually accepted in pediatric patients. This paper describes a laparo-scopic nephrectomy for an eight-month-old female with unilateral multicystic dysplastic kidney. The open surgical treatment for the asymptomatic patient with this renal condition is controversial problem. The laparoscopic nephrectomy, however, is recommended as a safe and useful procedure with minimal invasive access for children.
